Before you get started

Please read the following guidelines before continuing on to the Provider Profile form.

  • All required fields must be completed to submit your profile. In addition to your name, email, and the name of your practice, you will need your:
    • Service Address
    • Billing Address (if different) and Federal Tax ID
    • Continuing Education credits received within the past 24 months.
  • IMPORTANT! You will NOT be able to save your profile and come back later to finish, so please make sure you have all of the necessary information before getting started.
  • A signed CMEP Agreement Letter and completed Form W-9 will help CMEP process your application and can be uploaded from the Provider Profile. These documents are optional, but helpful.
